Quick Preparation Checklist
Personal and Contact Information
- Preferred name or nickname (this will be printed on your ID badge)
- Address and phone number
- Profile photo (youth participants)
- Parents / legal guardians (youth)
- Custody details and agreements (youth)
Health Information
- Medical conditions (ADHD, Asthma, eating disorder, emotional/behavioral issues)
- Recent health issues
- Any planned or anticipated medical procedures
- Doctor and dentist contact deails
- Current immunizations, date of last shots
Emergency Contacts
We will require two emergency contacts; for youth, these cannot be the parents.
- Name
- Phone numbers (home, mobile, work)
Medications and Treatments
- Brand name
- Reason for medication
- Dosage
- Schedule
- Possible side or adverse affects
Travel Information
- Passport number
- Passport date of issue and expiry
- Passport gender (Male / Female / X)
- Country of birth and current nationality
- Additional nationalities held (dual citizenships)
- EU / EEA / CH Citizenship
- Do you have a family member who is a citizen of CH or a EU/EEA country?
Allergies and Dietary Requirements
- Food-based allergy details
- Environmental allergies
- Medication allergies
- Severity, exposure steps, and if trace amounts are okay for each allergy
- Dietary needs or preferences
Mobility and Accessibility
- Mobility restrictions and devices
- Non-visible disabilities
- Special medical equipment needs
Jamboree Information
- Language proficiency for English, French, Arabic, and Spanish
- Activities preferences (comfort level)
- Jamboree shirt size (adults only)
Why do you ask for this information?
Learn more about how your medical information helps the Canadian Contingent and Jamboree prepare, respond, and support members safely at the 2027 World Scout Jamboree, and how your information will be used.
For Parents and Guardians
Youth forms may require parent or guardian contact information. If you have multiple participants, please prepare details for each participant to ensure a smooth process.
ePACT will reuse some information across family profiles, but each participant’s form must still be reviewed and submitted.
Adult members attending with a child will need to complete an adult ePACT form. Prepare your health, emergency, and travel information ahead of time to expedite the process.
ePACT will use the email address provided by the first parent / guardian identified during your initial registration to send the invitation.
We want to ensure that parent complete the process since you have access to the information required.
Why are you using the email address of the first parent/guardian?
We assume that the first parent entered on the registration form is the parent who completed the original registration process.
When ePACT sends our a link to complete the profiles, if multiple emails are included the first person to click on the link takes ownership of the process, and the other emails can no longer accept the invitation. This way we ensure that it’s a parent, and not a youth, who accepts the invitation.
You can add additional parents/guardians during the process, which will provide them with full access to ePACT to complete and update the information.

Streamline Your Process
ePACT offers the convenience of reusing some information across family profiles, saving you time. However, it’s crucial that each participant’s form is individually reviewed and submitted to ensure accuracy and completeness. This step is vital for maintaining the safety and preparedness of all members attending the World Scout Jamboree 2027.
